You can start learning about transition while you’re in middle school. These resources are not here to tell you what you should or need to do. These resources are here for you to explore and learn about what your options are so that you can choose your own path.
What is transition anyway? Transition planning is a process to help students with an individulized education plan (IEP) decide what they want to do after high school (understood.org).
Connections to transition specialists who provide individualized information, support, and referrals to self-advocates, youth, and parents by phone, email, and in-person.
